What it is, How it works

When taken close to the scalp, hair samples can offer nearly a 3-month detection period for drugs and alcohol. Hair collection is straightforward, relatively hard to tamper with, and is easily transportable.

A 1.5-inch sample consisting of about 200 hair strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is the ideal sample size for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, a 150mg specimen is preferable. It is suggested to use a jeweler’s scale for weighing the sample. If scalp hair isn't accessible, a similar quantity of body hair is acceptable. When referring to head hair, it specifically means scalp hair. Body hair refers to all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory's system. This step ensures the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and concludes any further data entry not covered by an electronic chain of custody system.

Screening involves a preliminary assessment for drug usage. While Screening is economical for ruling out drug use for most samples, a positive result from this stage requires confirmation to be legally valid. Samples with a presumptive positive result in Screening require further verification.

Once a sample is identified as presumptively positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. During Extraction, drugs are extracted from the hair at significantly lower concentrations than with other methods (for instance, urine or oral fluid testing), adding complexity to hair drug screening.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The full laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation guidelines.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
