A 1.5-inch collection of around 200 hair strands (roughly the dimension of a #2 pencil) near the scalp yields 100mg of hair, the ideal amount for analysis and verification. For EtG, additional tests, or evaluations exceeding 10 panels, 150mg of hair is advised. We suggest measuring the hair sample with a jeweler’s scale. If scalp hair is missing, a similar amount of body hair can be utilized. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The four main phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial integration of a sample into a lab's system. This involves confirming the sample was sealed and transported correctly, assigning it a random LAN (Laboratory Accessioning Number), and completing any other data input not provided by an electronic custody chain.

Screening provides a preliminary assessment for drug abuse. While it serves as a financially logical way to exclude drug presence in most samples, any positive findings require confirmation for court admissibility. Samples that show a presumptive positive in Screening necessitate additional verification.

If Screening uncovers a presumptive positive, additional hair is extracted from the initial sample for Extraction. Here, drugs are isolated from hair at concentrations lower than other methodologies (e.g., urine or oral fluid), making hair drug analysis the most challenging method to perform.

Confirmation of positive findings from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
