What it is, How it works

Hair testing has emerged as a robust method for detecting alcohol and drug use. Hair strands maintain a long-term record of substances by capturing biomarkers within their fibers as they grow. When hair is collected near the scalp, it can reveal a detection period of up to about 90 days for alcohol and other drugs. Hair collection is straightforward, hard to tamper with, and simple to transport.

Collecting a 1.5-inch segment of approximately 200 hair strands (similar to the girth of a #2 pencil) near the scalp provides a 100mg sample, which is optimal for screening and verification. For EtG, additional analyses, or tests with more than 10 panels, a 150mg sample is advisable. Using a jeweler’s scale to measure the specimen is recommended. If scalp hair is unavailable, a comparable amount of body hair may be collected. References to head hair denote scalp hair exclusively. Body hair encompasses other types such as facial or axillary hair.

Process Overview

The laboratory processing of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts the sample's journey through the laboratory's systems. This process includes checking if the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening can economically eliminate drug usage in most samples, a positive screen needs confirmation to be valid in legal matters. Samples indicated as positive in Screening require further confirmation.

Samples indicated presumptive positive during Screening have additional hair extracted from the original sample for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or saliva), which makes hair drug screening a particularly challenging process.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicated as positive undergo a cleansing process prior to confirmation, if necessary. This comprehensive lab procedure, from Accessioning to Confirmation, is evaluated following both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection window: Hair drug tests reveal drug use for up to 90 days, unlike urine tests with shorter detection time.
  • Difficult to manipulate: Cheating a hair drug test is extremely tough, leading to more reliable results.
  • Shows historical patterns: It provides a timeline of drug usage rather than just recent activity.

Limitations:

  • Recent use undetectable: Drugs become detectable in hair approximately 5-7 days post-use.
  • Expense: Hair drug tests typically cost more than other testing techniques.
  • Result variability: Factors like hair color and natural differences in hair growth can influence drug metabolite levels in the hair.

Note: The term "hair follicle tests" can be misleading as the analysis is conducted on the hair strand, not the follicle found below the scalp.
