By head hair, we mean only scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug testing process in a laboratory encompasses four critical stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a specimen is initially integrated into the laboratory system, ensuring it was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent in an electronic chain of custody.

Screening provides an initial rapid assessment for substance misuse. Though cost-efficient to dismiss drug use for most samples, Screening results that are positive require confirmation for legal validity. Any presumptively positive screenings necessitate a secondary confirmation.

If Screening yields a presumptive positive, additional hair is retrieved from the sample for Extraction. This phase extracts drugs at significantly lower levels from hair compared to other methods (e.g., urine, oral fluid), posing a greater challenge to execute hair drug screening.

Confirmation for any positive screening outcome is done through GC/MS, GC/MS/MS, or LC/MS/MS. Pre-confirmation, all presumptive positives undergo washing as necessary. The full laboratory process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
