However, for detecting drug use within five days, urine tests remain the most precise.

What it is, How it works

Hair testing is recognized as a robust mechanism for identifying drug and alcohol consumption. Hair captures long-term traces of alcohol and drugs by embedding biomarkers within the hair's growing fibers. A sample taken near the scalp can offer a detection period of up to roughly 3 months for substances like alcohol. Hair is easy to gather, challenging to tamper with, and straightforward to ship.

A 1.5-inch portion of roughly 200 hair strands (approximate diameter of a #2 pencil) located nearest to the scalp yields 100mg of hair, which is the perfect amount for both screening and confirmation. For EtG, additional tests, and/or analyses beyond 10 panels, a larger 150mg specimen is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn’t available, a similar quantity of body hair can be used. Head hair specifically refers to hair from the scalp, whereas body hair encompasses all other hair kinds (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves these four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is about the initial integration of a sample into the lab's infrastructure. It includes confirming that the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and doing any additional data entry not available via an electronic chain of custody system.

Screening comprises an initial rapid assessment for drugs of abuse. Although Screening is an economical method for eliminating drug consumption in most cases, a favorable screening result requires validation for legal proceedings. Any samples initially flagged positive in Screening demand secondary substantiation.

In cases of a tentative positive result from Screening, more hair is obtained from the original specimen for Extraction. During this phase, substances are extracted from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), rendering hair drug screening as the most complex to execute.

A confirmed positive screen is validated using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples preliminarily marked positive are cleansed before confirmation as necessary. The entire laboratory sequence from Accessioning to Verification is conducted under the CAP (College of American Pathologists) Hair designation and upholds ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
