The laboratory processing of a drug test result consists of four core steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial phase where a sample is incorporated into a laboratory’s system. This phase includes verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and entering any additional data not provided by an electronic chain of custody protocol.
Screening involves a preliminary assessment for drug abuse. Screening is a cost-efficient method for excluding drug use in most samples; however, a positive screen must be confirmed to be valid in legal situations. Samples that are initially positive in Screening require subsequent confirmation.
If a sample is initially positive in Screening, additional hair is taken from the original specimen and prepared for Extraction. Drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug testing a challenging methodology.
Confirmation of any positive screening outcome is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are cleansed before confirmation as required. The entire process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and compliance with ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
