A sample measuring 1.5 inches, comprising about 200 hair strands (approximately the width of a #2 pencil), collected nearest the scalp, will yield roughly 100mg of hair, which is the ideal quantity for analysis. For EtG, additional assessments, or tests exceeding 10 panels, a recommendation of 150mg is suggested. It is advised to use a jeweler’s scale for weighing the specimen. If scalp hair isn't available, a proportional amount of body hair can be used. When referring to head hair, it means only scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

Laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial integration into the lab's system. This covers confirmation that the sample is correctly sealed and transported, assignment of a random LAN (Laboratory Accessioning Number), and completion of remaining data entry not handled by an electronic chain of custody system.

Screening provides a preliminary assessment for drug use. Although economical for ruling out drug usage across most samples, any positive screen requires confirmation to be legally viable. Samples that show a presumptive positive in Screening require further confirmation.

For presumptive positive samples from the Screening phase, additional hair is extracted from the original sample for Extraction. In this stage, drugs are drawn from hair at notably lower concentrations compared to other methods (like urine or oral fluids), making hair drug testing the toughest methodology to execute.

Positive screening outcomes are confirmed using GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is scrutinized in accordance to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
