For tests involving EtG, add-ons, or more than 10 panels, 150mg is suggested. It is advisable to measure the sample using a jeweler’s scale. If scalp hair isn’t accessible, an equivalent amount of body hair can be used. When discussing head hair, it refers solely to scalp hair, while body hair encompasses all other types (like facial or axillary).

Process Overview

The key stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the sample's initial integration into a lab's system. This step verifies proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and completes data entry not included in an electronic chain of custody.

Screening offers a preliminary assessment for drug abuse. Though Screening is cost-efficient for ruling out drug use in most samples, a positive result requires confirmation for court acceptance. Samples presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests positive in Screening, additional hair from the specimen is prepared for Extraction. This stage involves extracting drugs from hair at concentrations significantly lower than other methods (like urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are washed before confirmation, as necessary. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
