A 1.5-inch segment encompassing around 200 hair strands (akin to the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen weight of 150mg is advisable. Using a jeweler’s scale to measure the specimen is recommended. If scalp hair is unavailable, a comparable amount of body hair may be used. When discussing head hair, we are specifically referring to scalp hair. Body hair encompasses all other hair types (such as facial and axillary hair).

Process Overview

The laboratory procedure for processing a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory's system. This step includes ensuring the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and accomplishing any additional data entry not supported by an electronic chain of custody system.

Screening represents an initial rapid evaluation for drug misuse. Screening serves as a cost-efficient approach to dismiss drug use in the majority of samples; however, any positive screen results require confirmation to be deemed valid in legal proceedings. Samples showing presumptive positive results during Screening require further verification.

If a sample is presumptively positive during the Screening phase, additional hair is extracted from the initial specimen for the Extraction process. During this phase, drugs are retrieved from hair at significantly lower concentrations than other methods (such as urine or oral fluid), highlighting why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is performed utilizing GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples tested as presumptive positive undergo washing prior to confirmation as deemed necessary. The entire laboratory workflow from Accessioning to Confirmation is audited in accordance with the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
