For tests involving EtG, additional compounds, or more than 10 panel drugs, 150mg of the sample is suggested. We recommend using a jeweler’s scale for specimen measurements. If scalp hair is not available, a similar quantity of body hair is acceptable. By head hair, we mean scalp hair only, while body hair includes all other types (such as facial or axillary hair).

Process Overview

The primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into the laboratory's system. This ensures the sample has been sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry absent in an electronic chain of custody system.

Screening consists of a preliminary rapid test for drugs. While Screening is cost-efficient for eliminating drug use in most samples, a positive screening must be verified to hold up in legal proceedings. Any samples that yield a presumptive positive in Screening require further confirmation.

If a presumptive positive is observed during the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening particularly challenging to execute.

Confirmation for any positive Screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The whole laboratory process from Accessioning to Confirmation adheres to both CAP (College of American Pathologists) Hair designation and accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
