For EtG, specialized assessments, and/or examinations exceeding 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not present, an equal load of body hair can be used. By head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory testing procedure for detecting drugs includes four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with integrating a sample into the lab's system. This step involves ensuring the sample was sealed and transported correctly, giving it a random LAN (Laboratory Accessioning Number), and adding any extra data not included in an electronic custody chain.

Screening serves as an initial evaluation for substance abuse. Although Screening is a budget-friendly method to exclude drug usage for most samples, any positive results require confirmation for court admissibility. Samples that test positive in Screening need a secondary validation.

A sample testing positive during Screening means more hair is taken from the initial batch for Extraction. This involves removing drugs from hair at concentrations lower than those in other testing methods (like urine or oral fluid), which is why hair drug testing is the most sophisticated technique.

Confirmation for any positive result from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleaned before confirmation as required. The whole lab process from Accessioning to Confirmation is examined per the CAP (College of American Pathologists) Hair specifications and complies with ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
