What it is, How it works

Hair testing is now recognized as an effective means for identifying drug and alcohol consumption. Hair offers an extended timeline of alcohol and drug use by embedding biomarkers within the strands of growing hair. When clipped near the scalp, hair can reveal up to a 3-month period of alcohol and drug usage. Hair collection is straightforward, less prone to tampering, and simple to transport.

A sample size of 1.5 inches containing around 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for analysis and verification. For EtG, additional tests, and/or panels exceeding 10, a quantity of 150mg is advised. We advise weighing the sample on a jeweler's scale. In the absence of scalp hair, an equivalent amount of body hair can be taken. By head hair, we mean only scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within a laboratory’s system. This step ensures the sample was sealed and dispatched correctly, assigns a unique LAN (Laboratory Accessioning Number), and fills in any missing information not submitted via an electronic chain of custody system.

Screening entails a preliminary rapid examination for drugs of abuse. Although Screening is an economical method to rule out drug use in many samples, a positive initial result must be verified to be legally recognized. Any samples that yield a presumed positive result during Screening require subsequent confirmation.

If the Screening indicates a presumptive positive result, additional hair is removed from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair with a lower concentration compared to other methods (e.g., urine or oral fluid), thus making hair drug screening one of the most challenging techniques to execute.

Any positive result detected in Screening is confirmed utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation if necessary. The full laboratory cycle from Accessioning to Confirmation is monitored under the CAP (College of American Pathologists) Hair criteria and ISO/IEC 17025 accreditation.
