Obtaining a 1.5-inch segment of roughly 200 hair strands (about #2 pencil width) nearest to the scalp ensures 100mg of hair, which is the perfect sample for both screening and verification. For EtG, additional features, and/or tests exceeding 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale for measuring the specimen is advisable. If scalp hair isn't available, an equal quantity of body hair can be used. "Head hair" refers exclusively to scalp hair, whereas "body hair" pertains to other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system, which includes confirming proper sealing and shipping of the sample, assigning a LAN (Laboratory Accessioning Number), and filling in any missing data not supplied by an electronic chain of custody system.

Screening offers an initial swift test for substances of abuse. Although Screening is cost-efficient and excludes drug use for most specimens, a positive outcome requires confirmation for court admissibility. Presumptive positive results need further validation.

In instances where a sample is presumptive positive at Screening, more hair is extracted from the original sample and prepared for Extraction. Here, drugs are extracted at a much lower concentration than with other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

The verification of any positive Screening result is conducted using GC/MS, GC/MS/MS or LC/MS/MS. Prior to confirmation, all samples indicating a presumptive positive are cleaned if necessary. The entire process from Accessioning to Confirmation is inspected following the CAP (College of American Pathologists) Hair protocol and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
