What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. Hair offers a comprehensive history of alcohol and drug use by capturing biomarkers within the fibers of the hair as it grows. When obtained near the scalp, hair can present a detection period of up to three months for alcohol and other drugs. The collection of hair is straightforward, challenging to tamper with, and convenient for transport.

A 1.5-inch section containing roughly 200 hair strands (around the size of a #2 pencil) closest to the scalp provides approximately 100mg of hair, which is the ideal amount for performing screenings and confirmations. For EtG, add-ons, and/or testing beyond 10 panels, a sample of 150mg is advised. We suggest using a jeweler’s scale for weighing the sample. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial integration into the laboratory's system. This stage involves confirming that the sample was appropriately sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not handled by an electronic chain of custody system.

Screening consists of a preliminary analysis for substances of abuse. Although Screening is a cost-effective approach for excluding drug usage in most samples, any positive results must be verified for court admissibility. Samples that are presumptively positive during Screening must undergo additional confirmation.

If a sample indicates a presumptive positive result during Screening, extra hair is extracted from the original specimen and readied for Extraction. At this phase, drugs are extracted from hair at a significantly lower concentration than alternative techniques (e.g., urine or oral fluid), rendering hair drug screening one of the most intricate methodologies available.

Confirmation of any positive findings from the screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
