What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol usage. By capturing biomarkers in the hair strands' fibers as they grow, hair delivers an extensive record of alcohol and other drug use. Hair collected near the scalp can reveal substance use within a roughly 3-month timeframe. It's easy to collect, hard to tamper with, and straightforward to transport.

To ensure a proper sample, collect a 1.5-inch length of about 200 hair strands (similar to the girth of a #2 pencil) from the scalp, which provides roughly 100mg of hair, perfect for both screening and confirmation. For EtG, additional tests, or beyond 10 panel screenings, 150mg is advisable. We suggest using a jeweler’s scale for accuracy. If scalp hair isn't available, the same amount of hair from the body can be used. "Head hair" refers only to scalp hair, while "body hair" encompasses other types like facial or axillary hair.

Process Overview

Laboratory drug test processing includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially integrated into a laboratory's system. This phase checks the sample's proper sealing and shipping, allocates a random LAN (Laboratory Accessioning Number), and completes any extra data documentation not provided by an electronic chain of custody framework.

Screening acts as an initial examination for drug indicators. As a cost-efficient method to dismiss drug presence in most cases, confirmed results from Screening are necessary for legal acceptance. Any sample suggesting a positive result requires further confirmation.

Should a sample appear positive in the Screening phase, additional hair is taken from the original sample for Extraction. This phase involves retrieving drugs at lower concentrations compared to other methods like urine or oral fluids, defining hair drug screening as the most intricate testing process.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. As needed, all initially positive samples are cleansed before confirmation. Every phase from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation criteria.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Forensic Applications and Interpretation: One of the vital objectives of hair testing is its application in forensic investigations and legal proceedings. The SoHT meeting in Verona emphasized the significance of rigorous interpretation of analytical results and the need for standardized protocols. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
