What it is, How it works

When gathered near the scalp, hair can offer a detection window for alcohol and other drugs spanning up to roughly 3 months. Hair is easy to gather, relatively hard to tamper with, and straightforward to transport.

A sample of 1.5 inches with roughly 200 strands of hair (akin to the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the ideal sample for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair may be used. Referring to head hair indicates scalp hair exclusively. Body hair signifies any other hair types (facial, axillary, etc.).

Process Overview

The four primary stages involved in processing a drug test result in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample into a laboratory’s system. This process includes verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening entails an initial quick inspection for drugs of abuse. While Screening is a cost-efficient way to dismiss drug usage for most samples, a positive screen must be validated to be permissible in court. Samples that are presumptively positive in Screening necessitate a follow-up confirmation.

Should a sample be presumptively positive at the Screening stage, more hair is extracted from the initial specimen and prepped for Extraction. In this step, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive screening result undergoes Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as required. The entire laboratory processing from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation complying with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
