What it is, How it works

Hair testing has emerged as a potent method for identifying drug and alcohol consumption. The hair encapsulates a prolonged history of substances by retaining biomarkers within the strands of hair as they grow. When gathered near the scalp, hair can offer an approximate 3-month detection window for alcohol and drugs. Hair collection is straightforward, relatively tamper-resistant, and easy to transport.

A sample measuring 1.5 inches with approximately 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp will yield around 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of specimen is advised. We suggest using a jeweler’s scale for measuring. If scalp hair is absent, a similar amount of body hair can be used. References to head hair specifically mean scalp hair. Body hair denotes other varieties like facial or axillary hair.

Process Overview

The laboratory procedure for drug testing encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary integration of a sample into a lab’s system. This step verifies correct sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and finalizes any unfinished data entry beyond what an electronic chain of custody system provides.

Screening is a prompt initial appraisal for detectable drugs. Though it is a cost-effective method to eliminate drug presence in most samples, a positive Screening necessitates confirmation for courtroom admissibility. All samples preliminarily positive in Screening must go through further Confirmation.

If results are presumptively positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this stage, drug extraction from hair occurs at much lower concentrations than other methods (like urine or oral fluids), making hair drug screening particularly challenging to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with provisional positive results are cleansed before Confirmation if necessary. The complete laboratory procedure from Accessioning through to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
