What it is, How it works

Hair testing has emerged as an effective instrument for identifying drug and alcohol consumption. Hair offers a prolonged record of alcohol and drug use by capturing biomarkers within the fibers of the hair strand as it grows. When obtained close to the scalp, hair can offer a detection window of up to about 3 months for alcohol and drugs. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advised. It's best to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, a comparable quantity of body hair can be collected. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, underarm, etc.).

Process Overview

The primary stages in the laboratory procedure of processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This process involves confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody system.

Screening encompasses an initial rapid test for drugs of abuse. While Screening provides a cost-effective means to eliminate drug use in most samples, a positive screen must be confirmed for court acceptability. Any samples that screen positively need an additional confirmation.

If a sample appears positive during the Screening phase, more hair is taken from the original specimen and prepared for Extraction. During Extraction, drugs are isolated from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to perform.

Confirmation of a positive Screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples confirmed to be presumptive positive are cleaned prior to confirmation as needed. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
