What it is, How it works

The fibers in a growing hair strand trap biomarkers, offering an extended timeline of drug and alcohol consumption. When extracted near the scalp, hair can unveil up to a 3-month period for detecting alcohol and other substances. Hair sample collection is straightforward, relatively tamper-resistant, and easy to transport.

A 1.5-inch hair sample containing roughly 200 strands (similar to the thickness of a #2 pencil) close to the scalp yields 100mg of hair, which is optimal for screening and verification processes. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is suggested. It's advisable to measure the sample weight using a jeweler's scale. In cases where scalp hair is unattainable, an equivalent volume of body hair can be gathered. The term "head hair" specifically refers to scalp hair, while "body hair" encompasses other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory evaluation for a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the preliminary treatment of a sample within a laboratory’s framework. This step ensures the sample is properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any supplementary data entry that may not be handled by an electronic chain of custody system.

Screening provides an initial, rapid assessment for the presence of drugs. As a cost-effective measure, it helps dismiss drug use in most samples; however, positive findings must be validated for court usage. Any samples that show presumptive positivity during Screening necessitate subsequent confirmation.

When a specimen tests presumptively positive during Screening, additional hair is retrieved from the original sample to prepare for Extraction. At this stage, drug substances are isolated from hair at considerably lower concentrations than in alternative methods (e.g., urine or oral fluid), rendering hair drug analysis the most intricate method to execute.

To authenticate any positive findings from Screening, assessments are conducted using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples deemed presumptively positive undergo washing before confirmation as required. Every segment of the laboratory process, from Accessioning to Confirmation, undergoes evaluation in accordance with the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
