Process Overview

The main components in processing drug test results in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample within the lab’s system. This involves checking that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary assessment to identify substances of abuse. Although screening is a cost-efficient method to exclude drug presence in most samples, a screen indicating a positive result must be confirmed to be legally valid. Samples presumptively positive in Screening do need further confirmation.

In the event of a presumptively positive result in the Screening stage, additional hair is retrieved from the original sample for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening a complex procedure.

Confirmation for any screen-positive result is performed with GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples are washed before confirmation, if needed. The whole laboratory procedure from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair specifications and the ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
