A hair sample measuring 1.5 inches of about 200 strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg of hair, which is ideal for screening and confirmation. For tests including EtG, add-ons, and/or those exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be utilized. References to head hair denote scalp hair exclusively, whereas body hair encompasses other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a laboratory’s system. This process checks the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not handled by an electronic chain of custody system.

Screening is a preliminary examination for substances of abuse. Although Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be confirmed for legal admissibility. Samples presumed positive during Screening necessitate further confirmation.

Should a sample test positive presumptively in the Screening phase, additional hair from the original specimen is prepared for Extraction. During this phase, drugs are extracted from hair at concentrations lower than other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation, as needed. The entire process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
