What it is, How it works

When taken near the scalp, hair can reveal drug and alcohol use for up to a 3-month period. Collection is straightforward, altering is challenging, and shipping is simple.

A 1.5-inch section comprised of around 200 hair strands (similar to the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a recommended 150mg of the sample is required. Using a jeweler's scale to weigh the specimen is advised. Should scalp hair be unavailable, an equivalent amount of body hair can be gathered. "Head hair" refers solely to scalp hair. "Body hair" includes any other hair types (facial, axillary, etc.).

Process Overview

The lab process for analyzing a drug test result comprises Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves adding a sample to the lab system. This entails checking if the sample was properly sealed and sent, assigning a LAN (Laboratory Accessioning Number), and performing any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary test for drugs of abuse. While it effectively rules out drug usage in most samples, a positive result necessitates confirmation for legal admissibility. Samples testing presumptively positive require secondary confirmation.

If Screening suggests a positive result, additional hair is taken for Extraction. At this stage, drugs are separated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), explaining why hair drug screening is challenging to carry out.

Confirmation of any positive results from Screening is handled via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo a wash before confirmation if required. The entire process from Accessioning to Confirmation is evaluated according to the CAP (College of American Pathologists) Hair criteria and in accordance with ISO / IEC 17025 standards.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
