What it is, How it works

Hair analysis is now recognized as a potent method for identifying drug and alcohol consumption. Hair offers a prolonged timeline of drug and alcohol usage by preserving biomarkers within the fibers of the developing hair strand. When cut close to the scalp, hair can provide an approximate 3-month detection period for alcohol and drugs. Hair is easy to gather, relatively resistant to tampering, and convenient to transport.

A 1.5-inch section consisting of roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for screening and validation. For EtG, supplementary tests, and/or assays exceeding 10 panels, a 150mg sample is advised. It's recommended to weigh the sample with a jeweler’s scale. If scalp hair is not obtainable, an equivalent quantity of body hair can be taken. When mentioning head hair, it solely refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure at the laboratory for processing a drug test result encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary incorporation of a sample into the laboratory system. This process confirms the sample's proper sealing and shipment, assigns it a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not available through an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. Although Screening is economical for excluding drug use in most samples, a positive screen requires confirmation for court admissibility. Any samples tentatively positive during Screening necessitate further confirmation.

If a sample tests tentatively positive in Screening, more hair is taken from the initial sample for Extraction. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening among the most challenging techniques to execute.

Verification of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples considered presumptive positives are cleansed before confirmation as required. The entire process—from Accessioning to Confirmation—is reviewed in alignment with both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
