A 1.5-inch section containing about 200 strands of hair (roughly the size of a #2 pencil) nearest the scalp will yield 100mg of hair, the optimal sample for evaluation and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We suggest weighing the sample on a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair can be collected. When discussing head hair, we are specifically referring to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four essential stages involved in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s system. This incorporates verifying that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any additional data entry not provided by an electronic chain of custody process.

Screening comprises an initial rapid assessment for drug abuse. Although Screening is an economical means to exclude drug usage in most samples, a positive screen must be confirmed to be admissible in legal proceedings. Any samples that test presumptively positive in Screening must undergo further confirmation.

If a sample tests presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are removed from hair at a much reduced concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening represents the most challenging method to conduct.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned prior to confirmation as required. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
