What it is, How it works

Hair analysis has emerged as an effective means for detecting drug and alcohol intake. Hair retains a comprehensive record of alcohol and drugs by embedding biomarkers within the hair strand fibers as they grow. When taken near the scalp, hair can offer as much as a 3-month detection period for alcohol and drugs. It is easy to collect hair, challenging to tamper with, and straightforward to send.

A 1.5-inch segment of approximately 200 hair strands (roughly the girth of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample for both testing and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for weighing the sample. Should scalp hair be unavailable, a similar quantity of body hair can be collected. In our context, head hair pertains exclusively to scalp hair. Body hair includes all other forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entry of a sample into a laboratory's system. This step involves confirming the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody system.

Screening performs an initial cursory check for substance abuse. While it is cost-effective for eliminating drug use in most samples, positive screenings must be substantiated for legal acceptance. Samples returning presumptive positive in Screening necessitate a follow-up confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is drawn from the original sample and prepared for Extraction. During this phase, drugs are extracted from hair at concentrations lower than in alternative methods (e.g., urine or oral fluid), hence hair drug testing is the most complex method to execute.

Confirmation of any positive screening finding occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo cleansing before confirmation as required. The whole lab process from Accessioning to Confirmation receives evaluation under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
