The process of hair collection is straightforward, challenging to tamper with, and simple to transport.

A sample of 1.5 inches, comprising approximately 200 hair strands (comparable in size to a #2 pencil) from closest to the scalp, provides 100mg of hair, which is perfect for analysis and confirmation. For EtG, specific add-ons, or tests exceeding 10 panels, a 150mg sample is preferred. Measuring the sample on a jeweler’s scale is advised. If scalp hair is unavailable, a comparable amount of body hair is acceptable. References to head hair indicate exclusively scalp hair, whereas body hair signifies all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of drug test outcomes involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introductory processing of a sample into a laboratory’s system, including the confirmation of proper sealing and shipping, the assignment of a unique LAN (Laboratory Accessioning Number), and the completion of any supplemental data input omitted by an electronic chain of custody system.

Screening pertains to an initial and rapid assessment for drug misuse. Though Screening is an economical approach to exclude drug use in most cases, a positive finding necessitates confirmation for legal acceptability. All samples flagged as presumptively positive during Screening require secondary validation.

When a sample turns presumptively positive in Screening, additional hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at considerably lower concentrations than other methods (e.g., urine or oral fluid), highlighting why hair drug analysis is exceptionally challenging.

The Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptively positive sample undergoes cleaning before confirmation, as needed. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
