What it is, How it works

Hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and other substances by embedding biomarkers within the fibers of the growing hair strand. When collected close to the scalp, hair can offer up to an estimated 3-month detection timeframe for alcohol and other substances. Hair is straightforward to collect, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (approximately the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the ideal sample for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent amount of body hair can be collected. When referring to head hair, we mean scalp hair only. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a laboratory’s system. This involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry missing from an electronic chain of custody system.

Screening involves an initial rapid check for substances of abuse. While Screening is an economical approach to exclude drug use in the majority of samples, a positive screen must be confirmed to be valid in court. Any samples that are presumptively positive in Screening do require a secondary confirmation.

If a sample is presumptively positive in the Screening phase, more hair is taken from the initial specimen for Extraction preparation. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methodologies (ex. urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The entire laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
