What it is, How it works

Hair analysis is now recognized as a robust method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug intake by embedding biomarkers within the strands of hair as it grows. When gathered near the scalp, it provides a detection timeframe of up to 3 months for alcohol and other substances. Collecting hair is straightforward, hard to tamper with, and convenient to transport.

A 1.5-inch sample from around 200 hair strands (roughly the diameter of a #2 pencil) close to the scalp yields 100mg of hair, which is the optimal amount for both screening and verification processes. For EtG, additional tests, and/or testing more than 10 panels, it is advisable to use 150mg of the sample. Using a jeweler's scale for weighing the specimen is suggested. If scalp hair isn't available, a similar quantity of body hair can be utilized. References to head hair specifically mean scalp hair. Body hair pertains to other types (such as facial, axillary, etc.).

Process Overview

The primary stages involved in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample within a lab’s system. This task includes confirming the sample was correctly sealed and sent, allocating a unique LAN (Laboratory Accessioning Number), and supplementing any further data entry absent from an electronic chain of custody system.

Screening involves an initial cursory assessment for drug abuse. This step is a cost-efficient method to exclude drug use in most samples, but any positive indication requires verification to hold up in legal settings. Samples that show a presumptive positive result in Screening need subsequent confirmation.

If a presumptive positive is noted in the Screening step, more hair is taken from the initial sample and readied for Extraction. During this step, drugs are obtained from hair in much smaller concentrations than alternative methods (urine or oral fluid, for example), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo a wash prior to confirmation if necessary. The complete lab protocol from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair certification and the ISO / IEC 17025 accreditation.

    The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
