What it is, How it works

Hair testing is recognized as an effective method for detecting alcohol and drug usage. Hair strands capture biomarkers within their fibers, offering a long-term substance use history. Hair collected near the scalp provides a detection period of up to three months for alcohol and other drugs. This method is simple to collect, challenging to tamper with, and easy to transport.

A 1.5-inch hair sample composed of around 200 strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is suggested. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair isn't accessible, an equal quantity of body hair can be used. The term head hair refers exclusively to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four main phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory system. This includes verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening entails a preliminary fast check for drugs of abuse. While Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be confirmed for legal admissibility. Any samples indicating positive in Screening necessitate a secondary confirmation.

If a sample is deemed positive during the Screening phase, additional hair is taken from the original specimen for Extraction. Drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methods to conduct.

Positive screening results are confirmed through GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are washed before confirmation as necessary. The laboratory process from Accessioning to Confirmation is reviewed under CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
