A 1.5-inch snippet of about 200 hair strands (approximately the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for evaluation and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of hair is advised. We suggest the specimen be weighed on a jeweler’s scale. If scalp hair is unavailable, a proportional quantity of body hair might be obtained. When we mention head hair, we specifically mean scalp hair. Body hair denotes all other types of hair (such as facial, axillary, etc.).

Process Overview

The four main phases in the lab processing of a drug analysis result include Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is initially processed into the laboratory’s framework. This entails ensuring that the sample was adequately sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not covered by an electronic chain of custody system.

Screening consists of a preliminary swift examination for banned substances. Although Screening is an economical method for excluding drug use on most samples, a positive screen necessitates confirmation to be legally valid. Any specimens that appear positive in Screening need a subsequent confirmation.

If a specimen is tentatively positive in the Screening phase, additional hair is drawn from the original sample and readied for Extraction. In this stage, drugs are removed from the hair at much lower concentrations than other methods (such as urine or saliva), which is why hair drug testing is one of the most challenging methodologies.

Confirmation of any positive screening result is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleaned before confirmation if necessary. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
