For EtG, additional tests, and/or tests involving more than 10 panels, 150mg of the specimen is advisable. Using a jeweler’s scale for weighing the specimen is recommended. If scalp hair is not present, an equivalent quantity of body hair can be collected. When discussing head hair, it pertains to scalp hair exclusively. Body hair encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample integration into a lab's system. This process involves confirming proper sealing and shipment of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing any further data entry not covered by an electronic chain of custody.

Screening offers a preliminary assessment for drug presence. Although it is a cost-effective measure to rule out drug usage in most samples, a positive result from screening requires confirmation for legal admissibility. Samples with presumptive positive in Screening must undergo a secondary confirmation.

In cases where a sample tests presumptive positive during Screening, more hair is extracted from the primary specimen for the Extraction process. Here, drugs are extracted from hair at a notably lower concentration than in other techniques (e.g., urine or oral fluid), rendering hair drug screening as the most challenging methodology to conduct.

Positive results identified in screening require verification via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
