When collected from the scalp, hair can offer a detection range of up to nearly 3 months for substances like alcohol and drugs. Collecting hair is straightforward, relatively resistant to tampering, and easily transportable.

A specimen measuring 1.5 inches consisting of roughly 200 hair strands (comparable to the diameter of a #2 pencil) from the scalp can yield 100mg of hair, which is ideal for both screening and confirming results. For EtG assessments, additional tests, or analyses exceeding 10 panels, it is best to use 150mg of the sample. We suggest using a jeweler's scale for accuracy in measuring the specimen. Should scalp hair be unavailable, an equivalent volume of body hair can be used. In references to head hair, we imply scalp hair exclusively. Body hair indicates any other hair types (e.g., facial, axillary).

Process Overview

The laboratory examination of a drug test result typically involves four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial phase of introducing a sample to the laboratory's system, which entails verifying the sample's proper sealing and shipping, allotting a LAN (Laboratory Accessioning Number) randomly, and completing any extra data not provided by the digital chain of custody methodology.

Screening performs an initial cursory review for substance abuse. Although Screening efficiently eliminates drug use in most samples, any positive screens must be validated to be considered valid legally. Any samples suggesting a positive result in Screening necessitate further confirmation.

Should a sample exhibit a tentative positive outcome in Screening, additional hair is extracted from the original sample and readied for Extraction. During this process, drugs are retrieved from hair at much lower density levels than with other methods (such as urine or saliva), which is why hair drug testing is notably more challenging to execute.

Confirmation for any positive screening is processed using GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples undergo cleaning before confirmation if needed. The complete lab procedure from Accessioning to Confirmation is conducted under the supervision of the CAP (College of American Pathologists) Hair category and ISO/IEC 17025 certification standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
