When gathered near the scalp, hair can offer a detection timeframe of up to roughly 3 months for alcohol and various drugs. Hair is straightforward to gather, somewhat challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches with approximately 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal quantity for assessment and verification. For EtG, supplementary tests, and/or examinations exceeding 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale to measure the sample. If scalp hair is not obtainable, an equivalent quantity of body hair can be collected. In reference to head hair, we mean scalp hair exclusively. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning means the initial intake procedure of a sample into a lab’s framework. This entails confirming that the sample was correctly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finalizing any extra data input not provided through an electronic chain of custody system.

Screening involves a preliminary rapid check for illicit substances. Although Screening is a budget-friendly means of negating drug consumption in most samples, a positive screen must be corroborated for court admissibility. Any samples suspected to be positive in Screening need secondary confirmation.

If a sample tests positive in the Screening phase, more hair is extracted from the original sample and prepped for Extraction. During this phase, drugs are extracted from hair at far lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples undergo washing before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair categorization and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
