A 1.5-inch segment comprising approximately 200 strands of hair (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect sample for testing and validation. For EtG, additional screenings, or tests exceeding 10 panels, 150mg of the sample is suggested. It is advisable to weigh the specimen on a jeweler’s scale. Should head hair be unavailable, a commensurate quantity of body hair is acceptable. The term 'head hair' pertains exclusively to scalp hair, whereas 'body hair' refers to all other types (facial, underarm, etc.).

Process Overview

The principal stages in the laboratory procedure for processing a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a specimen into a lab's system. This step ensures proper sealing and shipping of the specimen, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any remaining data entry not covered by an electronic chain of custody.

Screening consists of a preliminary rapid check for drug abuse. Although Screening is an economical approach for excluding drug use in most samples, a positive result must be verified to be valid in court. Samples testing presumptively positive in Screening require additional Confirmation.

Should a sample show positive in the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. During this phase, substances are isolated from hair at significantly lower concentrations than those found in other techniques (such as urine or oral fluid), making hair drug screening the most intricate methodology.

Confirmation of any positive result from screening is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that test positive initially are cleansed before Confirmation, as necessary. The complete lab process, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair specifications and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
