For EtG, add-ons, and/or extensive tests beyond 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unattainable, a corresponding volume of body hair is permissible. When discussing head hair, we mean scalp hair specifically. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four core stages in the laboratory handling of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into the lab's system. This step includes validating that the sample was correctly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data input absent from an electronic chain of custody framework.

Screening entails a swift preliminary assessment for substances of abuse. While economical for excluding drug use in most samples, a positive Screen must undergo Confirmation to be legally valid. Any samples presumptively positive in Screening must go through a secondary Confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for the Extraction phase. During Extraction, drugs are isolated from hair at concentrations much lower than other methods (e.g., urine or oral fluid), underscoring hair drug testing as the most complex method to perform.

Verification of positive screening outcomes is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleansed before Confirmation as necessary. Each procedural phase from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
