What it is, How it works

When taken near the scalp, hair samples offer a detection window of about three months for alcohol and drugs. Hair samples are easy to obtain, hard to tamper with, and simple to transport.

A sample approximately 1.5 inches in length, consisting of around 200 strands of hair (comparable to a #2 pencil) taken closest to the scalp, results in 100mg of hair, which is ideal for testing. For EtG, additional tests, or tests exceeding 10 panels, a 150mg sample is recommended. Using a jeweler’s scale for weighing is advised. If scalp hair isn’t accessible, a similar quantity of body hair can be used. "Head hair" refers specifically to scalp hair, whereas "body hair" covers all other types, like facial and axillary hair.

Process Overview

The laboratory processing of a drug test result mainly involves four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step in integrating a sample into a lab’s system. It includes verifying the sample’s proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for substance abuse, offering a cost-effective approach to discount drug use in most samples. However, a positive result requires further confirmation to be considered legally admissible. Samples testing positive during Screening require a secondary confirmation.

If Screening finds a sample presumptively positive, additional hair is extracted from the initial sample for the Extraction stage. Here, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging to perform.

Confirmation of a positive screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing before confirmation if necessary. The full process from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
