What it is, How it works

Hair captures a prolonged history of alcohol and drugs by securing biomarkers within the fibers of the growing hair. When collected near the scalp, hair can provide a detection window for alcohol and other drugs of up to around three months. It is easy to collect hair samples, challenging to tamper with, and convenient to ship.

A sample of 1.5 inches, consisting of approximately 200 hair strands (comparable to the size of a #2 pencil), nearest to the scalp will yield 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If obtaining scalp hair is not possible, a similar quantity of body hair may be collected. When discussing head hair, we refer specifically to scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves preparing a sample initially for integration into the laboratory’s system. This process includes verifying the sample's sealing and shipping were done properly, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry that an electronic chain of custody system does not provide.

Screening provides an initial rapid assessment for drugs of abuse. Although it is a cost-efficient method to dismiss drug usage for most samples, any positive screening must be verified to be valid in a legal context. Samples with presumptive positive results in Screening require further confirmation.

If the Screening results indicate a presumptive positive, additional hair is extracted from the original specimen to proceed to Extraction. Here, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Positive screening results undergo Confirmation through methods such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
