What it is, How it works

When cut close to the scalp, hair can give a detection range of up to three months for substances like alcohol and drugs. Hair is easy to collect, hard to tamper with, and simple to transport.

A 1.5-inch sample of about 200 hair strands (around the width of a #2 pencil) near the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, add-ons, or tests beyond 10 panels, it's best to collect 150mg of hair. We suggest weighing it on a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be used. By head hair, we mean only scalp hair. Body hair includes all other types (like facial and axillary).

Process Overview

The four central steps in lab-processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially integrating a sample into a lab system. This step ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and includes any extra data entry not addressed by an electronic chain of custody system.

Screening serves as a swift preliminary check for drug abuse. Screening is a cost-effective method to exclude drug use in most samples; however, for court admissibility, a positive screen requires confirmation. Any samples preliminarily positive in Screening need subsequent confirmation.

If Screening shows a preliminary positive, more hair is retrieved from the sample for the Extraction step. Here, drugs are removed from hair at a much lower concentration than in alternative methods (e.g., urine or oral fluids), making hair drug screening the most challenging method to execute.

Confirmation of any positive Screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo necessary washing before confirmation. From Accessioning to Confirmation, the lab process is evaluated under both the CAP (College of American Pathologists) Hair designation as well as ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
