For EtG, additional procedures, and/or tests exceeding 10 panels, 150mg of hair is advised. It's suggested to weigh the hair using a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair can be obtained. References to head hair specifically relate to scalp hair. Body hair encompasses all other hair types (beard, underarms, etc.).

Procedure Synopsis

The laboratory handling of a drug test result includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary integration of a sample into a laboratory system. This step involves confirming the sample was sealed and shipped appropriately, assigning a random LAN (Laboratory Accessioning Number), and completing any incomplete data entry absent from an electronic chain of custody system.

Screening entails an initial rapid assessment for drug abuse. Although it serves as an economical approach to rule out drug usage for most samples, any positive outcome requires confirmation to be deemed valid in legal settings. Samples that are presumptively positive in Screening necessitate further confirmation.

For samples that display presumptive positivity during Screening, more hair is extracted from the original sample for Extraction. At this phase, drugs are retrieved from hair at notably lower concentrations than other techniques (e.g., urine or oral fluids), which contributes to the complexity of hair drug screening processes.

Confirmation of positive screening results occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Samples showing presumptive positivity are cleaned before confirmation if needed. The full laboratory process from Accessioning through Confirmation is inspected with both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
