For EtG, additional tests, and/or evaluations over 10 panels, 150mg of the sample is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair cannot be obtained, a similar amount of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The core stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab system, which confirms the sample was appropriately sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody system.

Screening is the preliminary rapid assessment for drugs of abuse. Screening is a budget-friendly approach to exclude drug use in most samples; however, a positive result in Screening must be confirmed for legal admissibility. Any samples with a presumptive positive in Screening require secondary confirmation.

In the event of a presumptive positive result in Screening, more hair is extracted from the original sample in preparation for Extraction. In this stage, drugs are retrieved from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.

Positive screening results are verified through GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample is cleansed before confirmation as needed. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
