What it is, How it works

Hair captures a comprehensive, long-term record of substance use by embedding biomarkers within the structure of the hair as it grows. Gathered near the scalp, hair can offer a detection timeline of up to three months for alcohol and drugs. It's straightforward to collect, relatively hard to tamper with, and easy to transport.

A sample measuring 1.5 inches, comprising around 200 hair strands (similar in size to a #2 pencil), closest to the scalp will produce 100mg of hair, which is the optimal sample for testing. For EtG, additional analyses, or when testing beyond 10 panels, 150mg of the sample is suggested. It is recommended to weigh the sample using a jeweler's scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be used. The term "head hair" specifically denotes scalp hair only, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

There are four primary phases in the laboratory evaluation of a drug test sample: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary handling of a sample within a lab's system. This process includes confirming the sample was properly sealed and sent, ascribing a random LAN (Laboratory Accessioning Number), and fulfilling any additional data entry requirements not covered by an electronic chain of custody system.

Screening constitutes an initial brief assessment for substances of abuse. While Screening is an economical way to rule out drug usage for most samples, a positive screen must be validated for court acceptance. Any samples indicating a presumptive positive in Screening require a follow-up confirmation.

Should a sample yield a presumptive positive during Screening, further hair is extracted from the initial specimen and prepped for Extraction. Here, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or saliva), which is why hair drug testing is a more complex method to carry out.

Confirmation of a positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive readings undergo washing before final confirmation as necessary. The full laboratory procedure from Accessioning through Confirmation is examined under the CAP (College of American Pathologists) Hair specification and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
