For EtG, add-ons, and tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair might be collected. When we discuss head hair, we mean scalp hair specifically. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample in a laboratory's system. This process includes ensuring the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available through an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drug abuse. Screening offers a cost-efficient approach to exclude drug use across most samples, yet a positive screen requires confirmation to be legally acknowledged. Any samples appearing positive during Screening call for further confirmation.

Should a sample be apparently positive at the Screening step, additional hair is extracted from the initial sample for preparation in the Extraction stage. Here, drugs are isolated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging to execute.

Any positive screening outcome is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed prior to confirmation as needed. The comprehensive procedure from Accessioning through Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
