For EtG, supplementary, and/or more than 10-panel tests, a 150mg sample is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair is nonexistent, the same quantity of body hair is acceptable. By head hair, we refer strictly to scalp hair. Body hair includes all other varieties (beard, underarm, etc.).

Procedure Synopsis

The drug testing procedure in a lab comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample into the lab system. This process encompasses validating that the sample was adequately sealed and delivered, assigning an arbitrary LAN (Laboratory Accessioning Number), and finalizing any missing data input apart from that included in an electronic chain of custody.

Screening acts as an initial efficient evaluation for the presence of drugs. While Screening is a cost-saving method to identify negative drug use in most cases, a positive result necessitates confirmation to be valid in legal settings. Any samples with a tentative positive result in Screening require additional verification.

If detected positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a much subtler concentration than other approaches (such as urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive Screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a tentative positive result are cleansed before confirmation if necessary. The full laboratory sequence from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
