For tests like EtG or those exceeding 10 panels, 150mg is advised. Using a jeweler’s scale for measurement is recommended. If head hair is absent, a comparable amount of body hair may be gathered. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is comprised of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into the laboratory system, including verifying shipment integrity, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry beyond the electronic chain of custody system.

Screening involves a preliminary check for drug abuse. Although cost-effective for ruling out drug presence in most samples, a positive screening requires confirmation for court acceptability. Samples testing presumptively positive in Screening need additional confirmation.

For a Screening-flagged presumptive positive, more hair is extracted from the specimen for further Extraction. Drugs are retrieved from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening notably challenging.

GC/MS, GC/MS/MS, or LC/MS/MS confirm any positive screening results. Presumptive positive samples undergo washing before Confirmation as needed. The complete laboratory process, from Accessioning to Confirmation, adheres to CAP (College of American Pathologists) Hair designation and is accredited under ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
