What it is, How it works

Hair offers a prolonged history of substance consumption by capturing biomarkers within the fibers of the hair strand as it grows. When collected near the scalp, hair can reveal up to a 3-month detection period for alcohol and other drugs. Hair collection is straightforward, somewhat resistant to adulteration, and convenient to transport.

A 1.5-inch segment consisting of approximately 200 hair strands (similar to a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal amount for screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, 150mg of hair is advised. It's best to weigh the sample on a precision jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair may be used. Head hair specifically refers to scalp hair, while body hair includes all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample within a lab’s system. This process verifies that the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic custody chain.

Screening provides an initial swift assessment for drugs of abuse. Although Screening is cost-effective for eliminating drug use in most cases, a positive screen necessitates confirmation for legal admissibility. Samples showing presumptive positivity during Screening require further confirmation.

If a sample is positive during the Screening stage, additional hair is taken from the initial collection to prepare for Extraction. In this phase, drugs are extracted from hair at a much-diluted concentration compared to other methods (such as urine or oral fluid), highlighting hair drug screening’s complexity.

Confirmation of positive Screening outcomes is completed through GC/MS, GC/MS/MS, or LC/MS/MS. Samples showing presumptive positivity undergo washing before confirmation if needed. The entire laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
