Process Overview

There are four primary stages in the lab processing of a drug test sample: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the foundational processing of a sample within a lab's framework. This step involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and filling out any additional data entry not encompassed by an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of misuse. Samples that test presumptively positive in Screening require further confirmation.

If a sample tests as potentially positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are drawn from hair in lower concentrations compared to other methods (e.g., urine or oral fluid), explaining why hair drug screening is the most intricate methodology to apply.

Positive screening results are verified via GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed to be positive are washed as necessary before confirmation. The complete lab process from Accessioning to Confirmation undergoes review per both the CAP (College of American Pathologists) Hair guidelines and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
