Unlike other methods, hair captures a long-standing record of alcohol and drug use by embedding biomarkers within the fibers of its growing strands. When a sample is taken near the scalp, it can provide a detection period of up to three months for substances. Collecting hair is straightforward, it's relatively resistant to tampering, and can be easily mailed.

A sample size of 1.5 inches consisting of approximately 200 hair strands (comparable to a #2 pencil's thickness) closest to the scalp yields 100mg of hair, which is optimal for screening and verification. For tests involving EtG, add-ons, or more than 10 panels, a 150mg hair specimen is suggested. It's advisable to weigh the sample with a jeweler’s scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be utilized. References to head hair pertain specifically to scalp hair, while body hair encompasses all other types (like facial or axillary hair).

Process Overview

The laboratory processing of drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into the laboratory's system. This consists of ensuring the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent in an electronic chain of custody arrangement.

Screening serves as an initial rapid assessment for substances of abuse. Although Screening is a cost-effective strategy to exclude drug use in most samples, a confirmed positive is essential for legal admissibility. Samples appearing presumptively positive during Screening mandate further confirmation.

In cases where Screening results are presumptively positive, additional hair is selected from the initial sample and prepared for Extraction. This phase involves extracting drugs at notably lower concentrations from the hair compared to other methodologies (e.g., urine or oral fluid), which is why conducting hair drug screening is particularly challenging.

Confirmation for any positive Screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation as well as accreditation in accordance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
