For EtG, additional assays, and/or tests exceeding 10 panels, a 150mg sample is advisable. It's recommended to ascertain the sample's weight using a jeweler’s scale. In absence of scalp hair, an equivalent quantity of body hair can be sourced. The term head hair pertains solely to scalp hair, whereas body hair includes other types such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial appointment of a sample into a lab’s system. This involves confirming the sample’s sealing and shipment, allotting a unique LAN (Laboratory Accessioning Number), and performing any supplementary data entry absent from an electronic chain of custody.

Screening consists of an initial rapid appraisal for drugs of abuse. A Screening test is a cost-effective approach to dismiss drug use in most samples, though a positive screen mandates confirmation for legal admissibility. Confirmation of any positive Screening outcome is performed with GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptive positive are pre-washed before confirmation as required. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
