A sample size of 1.5 inches, consisting of around 200 hair strands (similar in size to a #2 pencil) close to the scalp, yields 100mg of hair, which is ideal for both screening and confirmation tests. For EtG, additional tests, and panels over 10, 150mg is advisable. We suggest weighing the specimen on a jeweler’s scale. Should scalp hair not be available, an equivalent quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair includes all other types, like facial and axillary hair.

Process Overview

The laboratory processing of a drug test result consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is integrated into a lab's system. It includes checks to ensure the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not captured by an electronic chain of custody system.

Screening is a preliminary swift review for drug abuse. Although it is an affordable method to rule out drug use in most samples, a positive screen must be confirmed to be legally valid. Any samples flagged as positive in Screening require further confirmation.

Should a sample show positive results in the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. During Extraction, drugs are isolated from hair at concentrations lower than other methods (such as urine or oral fluid), making hair drug screening the most challenging type to conduct.

Confirmation of any positive results from screening is done using GC/MS, GC/MS/MS or LC/MS/MS. All samples testing positive in screening are cleaned before confirmation as necessary. The full lab procedure from Accessioning through to Confirmation is carried out under the CAP (College of American Pathologists) Hair criteria and in compliance with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
