What it is, How it works

When obtained near the scalp, hair delivers a detection range of up to roughly 3 months for drugs and alcohol. Hair is straightforward to collect, slightly challenging to tamper with, and easy to send.

A 1.5-inch section of approximately 200 hair strands (akin to the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for testing and verification. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the hair is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, an equivalent volume of body hair can be gathered. When discussing head hair, it specifically pertains to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory evaluation of drug test outcomes entails four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory processing stage of a sample into a lab's system. This step verifies that the sample was securely sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any missing data entry not included in an electronic chain of custody framework.

Screening offers a preliminary review for drug abuse. While Screening is a budget-friendly strategy to exclude drug use in the majority of samples, a positive result necessitates more confirmation to be court-worthy. Any samples marked presumptively positive in Screening require further confirmation.

Should a sample appear presumptively positive during the Screening phase, additional hair is taken from the initial sample and readied for Extraction. During this phase, drugs are isolated from hair at significantly reduced concentrations than with other techniques (such as urine or oral fluid), which makes hair drug testing the most challenging technique to execute.

Confirmation of any positive screening outcomes is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All samples noted as presumptive positives undergo washing prior to confirmation as needed. The full laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
